- Legible Copy of Operating Authority
- Completed W‐9 Tax Form
- Insurance Certificate
- Copy of Driver’s License
Expectations
Once approved as a Carrier with PAM Transport, the Carrier and Recruiter will coordinate a date when truck is ready. Once the Carrier is available, they will be contacted by Operations within 2 hours. Operations will give instruction on where to pick up first trailer and load. The Carrier will deliver the load and submit bills. The Carrier receives payment within 48 hours upon delivery (if QuickPay selected).
